OHIP billing code for phone visit
Updated October 26, 2026
For a patient you already have a relationship with, bill the usual visit code (for example A007A) with the telephone indicator K301A; OHIP pays 85% of the in-person fee (95% for K005, K007, K197 and K198). For a patient with no relationship, bill A102A, limited virtual care by telephone, $15.00.
| Code | What it is | Fee | When to bill it |
|---|---|---|---|
| K301A | Telephone modality indicator | $0.00 | On every comprehensive virtual care claim done by phone, beside the visit code. It has no fee of its own. |
| A007A | Intermediate assessment | $44.55 | The most common phone visit for an existing patient. Paid at 85% by phone. |
| A001A | Minor assessment | $26.80 | A brief phone visit about one problem with an existing patient. Paid at 85% by phone. |
| K005A | Primary mental health care, per unit | $80.00 | Mental health care by phone, 20 minutes or more per unit. Paid at 95% by phone. |
| A102A | Limited virtual care by telephone | $15.00 | A phone visit with a patient who has no existing relationship with you. |
How to bill it
First check the relationship. Comprehensive virtual care needs an existing patient-physician relationship: an in-person insured service by you in the past 24 months, or the patient is enrolled to you or another physician in your group (A66).
With a relationship, bill the in-person code at its full value and add K301A on the same claim. OHIP reduces the payment to 85%, or 95% for K005, K007, K197 and K198, and shows explanatory code B2 on the remittance advice (A70). Only codes on the Appendix J 'video or telephone' list can be billed by phone (AJ1).
Without a relationship, bill A102A ($15.00). It must include a history, a medically appropriate exam, a diagnosis and a management plan (A73). Limited virtual care doesn't create a relationship.
Chart that the visit was by telephone, and record start and stop times for time-based codes. Only time spent talking directly with the patient counts (A68, A69).
Billing rules
- The visit must be patient-initiated, or a medically necessary follow-up to a patient-initiated visit. Check-in calls, administrative calls and calls to give routine normal results are not payable (A68).
- Not payable if the problem can't be managed properly without an in-person exam; then only the in-person service is paid (A67).
- General assessments (A003, A004) and all consultations are not on the telephone list, so they can't be billed by phone. Consultations and C- and W-prefix codes are video only (AJ1, AJ2).
- The patient and physician must both be in Ontario (A69).
- If the call switches to video, bill the modality used for more than half of the visit (A69).
- You or your group must offer an in-person visit in a clinically appropriate time if one is needed or requested (A69).
- FHO: Q012A after-hours premium can go on comprehensive virtual services for enrolled patients (AQ11). Under FHO+, phone care to enrolled patients counts as direct patient care for the Q310A hourly rate when you are in the office, or Q311A ($68 an hour) when you are not (Agreement p.61 §6.1).
Common rejections
- AT3: Billed as comprehensive virtual care (K300A or K301A), but OHIP found no existing patient-physician relationship: no in-person insured service with you in the past 24 months and no enrolment to you or your group (A66). Otherwise bill limited virtual care, A101A or A102A.
- AT1: OHIP's description is 'only one modality allowed'. Put either K300A or K301A on the claim, not both.
