Explanation

Updated October 26, 2026

K301A is the no-fee indicator that marks a comprehensive virtual care claim as done by phone. Phone visits pay 85% of the in-person fee (95% for some).

What it covers

K301A is not a service on its own. It is the modality indicator required on every comprehensive virtual care claim done by telephone (synchronous audio only). Bill the Appendix J Section 1 code at its in-person value and add K301A (A67, A70).

Comprehensive virtual care needs an existing patient-physician relationship, such as an in-person insured service by the same physician in the past 24 months, or enrolment to that physician or their group (A66, A67).

Billing rules

  • Telephone pays 85% of the in-person fee (95% for K005, K007, K197 and K198). Bill the full value; OHIP reduces it (A70).
  • Only codes on the Appendix J 'video or telephone' list can be billed by phone. Consultations and C-prefix and W-prefix codes are video only (AJ1, AJ2).
  • The chart must record that the visit was by telephone (A68).
  • The visit must be patient-initiated or a medically necessary follow-up. Check-in calls, administrative calls and routine normal results are not payable (A68).
  • If a call switches to video, only the modality used for more than 50% of the time is payable (A69).
  • The patient and physician must both be in Ontario (A69).
  • For time-based codes, only time in direct communication with the patient counts (A69).

Often billed with

  • Appendix J Section 1 'video or telephone' codes such as A007A, A001A, K005A, K007A and K013A (AJ1).
  • E079 smoking cessation and age premiums are payable on phone visits; E080 and K189 are video only (A74).
  • FHO: Q012A after-hours premium on comprehensive virtual services (AQ11, information only).

Common rejections

  • EH2: The health card version code doesn't match the Ministry's record. Get the current version code and resubmit.
  • VH8: The date of birth doesn't match OHIP's record for the health number. Correct it and resubmit.
  • EQE: The physician wasn't registered in the group on the claim on the service date. A registration issue, not a coding one.
  • VJ7: OHIP received the claim more than three months after the service date (GP9). If it was first sent on time, use the stale-dated process.
  • EQ2: The specialty code on the claim wasn't registered and active for the physician on the service date. Correct it, or update the registration, then resubmit.
