Error code
E2 — Incorrect version code for service date
Wrong version code
The health card version code on the claim wasn't valid for the patient on the service date; the patient is often still covered.
What to do: Get the patient's current card or run a health card validation, then use the version code valid that date.
- Code
- E2
- Description
- Incorrect version code for service date
Explanation
Updated October 26, 2026
E2 means the health card version code on the claim wasn't valid for the service date. Get the right version code for that date.
What it means
E2 appears on the Remittance Advice (RA) when the version code on the claim doesn't match the one OHIP has for the patient on the service date. The two-letter version code changes when a new card is issued, while the 10-digit health number stays the same.
The patient is often still covered. The claim usually pays once it carries the version code that was valid on the service date.
Common causes
- The patient had a new card issued and the chart has the old version code
- The new version code was used for a service before the new card was issued
- A typing error in the version code
How to fix it
- Ask the patient for their current card, or run a health card validation check, and compare the version code with the claim.
- Update the patient's chart.
- If you can't get the card from the patient, physicians registered as health information custodians can request the health number and version code from ServiceOntario's Health Number Release service (the 24x7 Help Desk or form 1265-84).
- Send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the correct version code, or follow the instructions on the RA for that line.
Don't resubmit if
- You haven't found a different version code. Sending the same one again will fail again.
- The claim is already on your RA. A new claim for the same service will be treated as a duplicate. Use an RAI instead.
