Error code
EQD — Group inactive on service date
Group not active
The group number exists in the Ministry's records but wasn't active on the service date.
What to do: Check the number against the group's registration letter; if wrong, resubmit under the correct active group or solo number.
- Code
- EQD
- Description
- Group inactive on service date
Explanation
Updated October 26, 2026
EQD means the group number on the claim wasn't active with the Ministry on the service date.
What it means
OHIP checks that the group number on each claim is registered and active on the service date. EQD means the group exists in the Ministry's records but wasn't active that day. EQC is the related code for a group number that isn't registered at all.
Because the check is on the group, every claim sent under that group number for the same dates usually rejects the same way.
Common causes
- Claims sent under a new group before its registration start date.
- A group that closed, merged or changed models, with claims still going out under the old number.
- A typo in the group number that happens to match an inactive group.
- Billing software still set to an old group number.
How to fix it
- Check the group number on the claim against the group's registration letter.
- If the number was wrong, resubmit under the correct active group number, or the physician's solo number if they bill solo.
- If the number is right, ask the Service Support Contact Centre (1-800-262-6524) to confirm the group's effective dates. Have the group's registration corrected if needed. Since April 1, 2026, this is done through the Ministry's online registration application.
- Resubmit once the Ministry shows the group active on the service date. Older claims may need to go as stale-dated.
Don't resubmit if
- Nothing has changed: the group is still inactive on the service date and you're using the same number.
- The group has closed for that period. Send the claims under the number the physician was actually registered with.
How APEX handles it
For family practice claims, APEX resubmits EQD claims as billed, with the patient's date of birth taken from a health card check. Resubmitting only helps once the Ministry's group record is corrected, so confirm that before approving. See how APEX works · Pricing
