Code
EQF
Description
Provider inactive in this group on service date

Explanation

Updated October 26, 2026

EQF means the physician is on file as a member of the group on the claim, but the membership wasn't active on the service date.

What it means

Each group claim carries a group number and the billing physician's number. OHIP checks that the physician is registered as an active affiliate of that group on the service date.

With EQF, the Ministry knows the physician is linked to the group, but the affiliation hadn't started yet or had already ended on that date. EQE is the related code for a physician who isn't registered with the group at all.

Common causes

  • A new physician billed before their group membership start date.
  • A physician who left the group billed under the old group number.
  • A locum billed under the group outside their registered dates.
  • The membership change form was sent late, or with the wrong effective date.

How to fix it

  1. Confirm the physician's affiliation start and end dates with the group and the Service Support Contact Centre (1-800-262-6524).
  2. If the service was outside the membership dates, resubmit under the correct group number, or the solo billing number if the physician bills solo.
  3. If the dates on file are wrong, have them corrected. Since April 1, 2026, group membership changes go through the Ministry's online registration application. Resubmit after the change is processed.
  4. Watch the three-month submission window. Older claims go in as stale-dated.

Don't resubmit if

  • The affiliation dates haven't been corrected and you're sending the same group number again.
  • The physician genuinely wasn't a member on that date. Bill under the group or solo number they were registered with.
