Error code
I3 — Fee code not on the IHF licence on service date
Not on facility licence
The fee code isn't on the facility's licence profile (formerly IHF, now ICHSC) for the service date, so it wasn't paid.
What to do: Check the fee code and facility number, and ask the facility which services its licence covered on that date.
- Code
- I3
- Description
- Fee code not on the IHF licence on service date
Explanation
Updated October 26, 2026
I3: the fee code isn't on the facility's licence profile (formerly IHF, now ICHSC) for the service date, so it wasn't paid.
What it means
I3 means the fee code billed is not on the licence profile of the Independent Health Facility (IHF) for the date of service, so OHIP did not pay it. The Ministry now calls these facilities Integrated Community Health Services Centres (ICHSC).
Common causes
- The facility is not licensed for that service on that date
- The wrong fee code or facility number on the claim
How to fix it
- Check the fee code and facility number on the claim.
- Ask the facility to confirm which services its licence covered on the service date.
- If the service was covered by the licence, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
