OHIP billing code for family meeting
Updated October 26, 2026
The OHIP billing code for a family meeting is usually K002A, interview with relatives or the patient's substitute decision-maker, $80.00 per half-hour unit, billed under the patient's health number. It needs a separately booked appointment of at least 20 minutes and isn't payable when the only purpose is to get consent.
| Code | What it is | Fee | When to bill it |
|---|---|---|---|
| K002A | Interview with relatives or a person authorized to make treatment decisions for the patient, per unit | $80.00 | Medically necessary information can't be obtained from or given to the patient, for example because of illness or incompetence, and the purpose isn't only consent. |
| K003A | Interview with the Children's Aid Society or legal guardian on behalf of the patient, per unit | $80.00 | The interview is with the CAS or a legal guardian, for a purpose other than getting consent. |
| K015A | Counselling of relatives on behalf of a catastrophically or terminally ill patient, per unit | $80.00 | You counsel one or more relatives about a catastrophically or terminally ill patient. Pre-booked counselling. |
| K004A | Family psychotherapy, 2 or more family members present, per unit | $86.85 | The meeting is psychotherapy with at least two family members present at the same time. |
How to bill it
Pick the code by what the meeting is. Gathering information from, or giving information to, relatives or the substitute decision-maker because the patient can't: K002A. Counselling relatives of a catastrophically or terminally ill patient: K015A. Psychotherapy with two or more family members present: K004A.
Bill under the patient's health number and the patient's diagnosis (A28, GP60).
Count units by time: 1 unit is at least 20 minutes, 2 units at least 46, 3 units at least 76, 4 units at least 106, then one more unit for each 30 minutes. Chart the start and stop times, or the service isn't payable (GP7, GP55).
If the meeting is by video or phone with a patient you have an existing relationship with, add K300A (video) or K301A (phone) (AJ1, A70).
Billing rules
- K002 must be a booked, separate appointment of at least 20 minutes (A28).
- K002 is not payable when the information obtained is part of the history normally taken in a consultation or assessment (A28).
- K002 is payable only if you can show the interview was not solely to obtain consent (A28).
- K002 and K003 apply when medically necessary information can't be obtained from or given to the patient or guardian, for example because of illness or incompetence (A28).
- Interviews must be rendered personally by the physician (GP61).
- K015 is counselling, so it must be pre-booked or OHIP pays it as a lesser assessment; only the services on the GP58 list (for example K002, K003, E080) are payable the same day by you (GP58).
- K004 outside hospital isn't payable with a consultation or assessment by you the same day unless the diagnoses are clearly different (A24).
- FHO: K002A is an in-basket core service (Agreement Schedule 2, p.79).
Common rejections
- AT3: No patient-physician relationship. The meeting was billed as video or phone care (K300A/K301A) but OHIP found no in-person service in the last 24 months and no enrolment to you or your group (A66).
Questions
What is the OHIP billing code for family meeting?
The OHIP billing code for a family meeting is usually K002A, interview with relatives or the patient's substitute decision-maker, $80.00 per half-hour unit, billed under the patient's health number. It needs a separately booked appointment of at least 20 minutes and isn't payable when the only purpose is to get consent.
Can I bill a family meeting with the hospital team or home care?
A meeting with other physicians or regulated health professionals about the patient is a case conference, not K002. The Schedule has separate case conference codes, such as K121A for a hospital in-patient and K700A for a palliative care out-patient, each with at least 2 other participants and a 4-per-12-months limit (A33).
Can I bill an assessment of the patient on the same day?
Yes, if the assessment is a separate service. K002 can't pay for information you'd normally collect as part of that assessment's history (A28).
