It is paid in addition to the assessment fee, where one applies (A58).
Billing rules
- One per Ontario Health atHome home care program admission per patient (A59).
- Not payable if the patient is already receiving Ontario Health atHome home care (A59).
- Pays nil if the form is completed in whole or in part by anyone other than the physician or the physician's employee (A58).
- Not on the counselling same-day list, so it isn't payable the same day as K013 or other counselling by you (GP58).
- Unlike K071 and K072, it is not barred in a week when G512 is billed (J102).
- FHO: on the long-term care base-rate list (Agreement Schedule 2A, p.85) but not the community in-basket list (Schedule 2, p.79).
Often billed with
- A007 or another assessment at the same visit; K070 is paid in addition to the assessment fee (A58).
- K071 acute home care supervision during the first 8 weeks after admission to home care (A59).
Common rejections
- V21: The claim had no diagnostic code. Add the three-digit code for the condition that needs home care and resubmit.
