Explanation

K300A is the no-fee indicator that marks a comprehensive virtual care claim as done by video. Required on the claim; video pays the full in-person fee.
What it covers
K300A is not a service on its own. It is the modality indicator required on every comprehensive virtual care claim done by video. Bill the Appendix J Section 1 code at its in-person value and add K300A (A70).
Comprehensive virtual care needs an existing patient-physician relationship: an in-person insured service by the same physician in the past 24 months, enrolment to that physician or their group, or certain consultation routes (A66, A67). Video means two-way synchronous video on an Ontario Health verified platform (A67).
Billing rules
- Video services are paid at the in-person fee (A70).
- Only Appendix J Section 1 codes can be billed by video. Consultations and C-prefix and W-prefix codes are video only (AJ1, AJ2).
- The platform must be on Ontario Health's verified list, and the chart must record that the visit was by video (A68).
- The visit must be patient-initiated or a medically necessary follow-up. Check-in calls, administrative calls and routine normal results are not payable (A68).
- If the visit switches between video and phone, only the modality used more than 50% of the time is payable (A69).
- The patient and physician must both be in Ontario (A69).
- The physician or group must offer an in-person visit in a clinically appropriate time if needed or requested (A69).
Often billed with
- Appendix J Section 1 codes such as A007A, A001A, K005A and K013A (AJ1).
- Virtual care premiums such as E079, E080 (video only) and age premiums (A74).
- FHO: Q012A after-hours premium on comprehensive virtual services (AQ11, information only).
Common rejections
- VH8: The date of birth doesn't match OHIP's record for the health number. Correct it and resubmit.
- EH2: The health card version code doesn't match the Ministry's record. Get the current version code and resubmit.
- EQE: The physician wasn't registered in the group on the claim on the service date. A registration issue, not a coding one.
- R04: OHIP's description is 'service excluded from RMBS', the reciprocal billing system for patients insured by another province. The service can't be billed through reciprocal billing for that patient.
- EH4: The patient's OHIP coverage had ended before the service date. Confirm the health number, then the coverage.
