Best OHIP billing software in Ontario, compared (2026)

Ontario physicians can bill OHIP with software they run themselves, with an OHIP billing agent who does it for them, or with a mix. This page puts APEX next to nine other Ontario options, including Dr. Bill (which now includes MDBilling), CabMD and ClinicAid, on price, free trials, billing agents, phone apps, rejected claims and FHO+, using what each company publishes on its own website.

We make APEX, so it's listed first. Every other row links to the vendor's own pages, and each vendor has its own page here with when it may suit you better than APEX.

What each one costs

Monthly price before tax at what OHIP pays you a month. DoctorCare doesn't publish prices; ClinicAid prices by claims sent a month.

Questions

What is the best OHIP billing software?

It depends on what you want to pay for. For the lowest flat price: APEX (about $13 a month yearly), OHIPay ($20 a month) or CabMD ($40 a month). For a capped percentage: SnapBill, IntelAGENT or MedConcierge. For an OHIP billing agent who does the work: Dr. Bill Comprehensive, CabMD Full Service, IntelAGENT Premium, OpsMed or DoctorCare. For fixing rejected, underpaid and stale-dated claims and tracking FHO+, roster and preventive care bonuses: APEX.

Is there free OHIP billing software?

Not permanently, but most have a free trial: OHIPay 6 months, SnapBill and IntelAGENT and MedConcierge 3 months, CabMD 60 days, Dr. Bill 2 months on Comprehensive, ClinicAid 30 days. APEX has a free plan with 5 minutes of use every 3 days and no time limit.

How much does Dr. Bill cost?

Dr. Bill charges a percentage of what OHIP pays you: 0.25% on Essentials and 1.95% on Comprehensive, which includes a billing agent. At $30,000 paid a month that's about $75 or $585 a month. Agent help on Essentials is $50 an hour. Group pricing is by quote.

How much does CabMD cost?

CabMD Self Service is $40 a month per physician at any volume, with a 60-day free trial. Full Service, run by JCL billing agents, is 1.95% of paid claims.

Dr. Bill vs CabMD: which costs less?

It depends on your billings. CabMD Self Service is a flat $40 a month; Dr. Bill Essentials is 0.25% of paid claims, so it costs less than CabMD until you're paid about $16,000 a month and more after that. Their agent plans (Dr. Bill Comprehensive and CabMD Full Service) are both 1.95%. APEX Essential is about $13 a month on the yearly plan.

How much does ClinicAid cost?

ClinicAid charges by claims sent a month: $27 for up to 100, $49 for 250, $99 for 700 and $179 for 1,500, with custom pricing above that. Its Error Correction add-on is $150 a month per physician. There's a 30-day free trial.

What happened to MDBilling?

RBC bought MDBilling.ca in October 2022 and merged it into Dr. Bill, which RBC already owned. mdbilling.ca now redirects to Dr. Bill, so MDBilling vs Dr. Bill is now one product: compare APEX vs Dr. Bill.

What's a good Dr. Bill alternative?

If you want a lower or fixed price, APEX, OHIPay and CabMD charge a flat fee instead of 0.25% to 1.95% of paid claims. If you want an agent to do the work, CabMD Full Service, IntelAGENT Premium, OpsMed and DoctorCare are agent services. See APEX vs Dr. Bill for a side-by-side.

Which OHIP billing app works on an iPhone?

Dr. Bill, CabMD and IntelAGENT have native OHIP billing apps on the App Store and Google Play. APEX runs in the phone's browser, adds to the Home Screen with Face ID sign-in, and scans Ontario health cards.

Should I use an OHIP billing agent or billing software?

A billing agent or medical billing service in Ontario does the work for a percentage, usually 1.5% to 2% of paid claims (OpsMed, Dr. Bill Comprehensive, CabMD Full Service, IntelAGENT Premium), or an unpublished subscription (DoctorCare). OHIP billing software costs less and keeps you in control; APEX also works out the fixes for you to approve.

Is there AI for OHIP billing?

APEX is built by Verismo AI. Instead of a billing agent, its Correction Centre works out the fix for each rejected claim automatically and finds underpaid claims; you approve each one before it goes to OHIP.

Can billing software find OHIP underbilling?

Some can. APEX's claim adjustments find paid claims billed below what they qualified for. Dr. Bill suggests codes and premiums, SnapBill suggests missed add-on codes, and OpsMed and DoctorCare look for missed codes as part of their service.

Where can I look up OHIP billing codes for free?

APEX's resources list every OHIP fee code with its July 2026 fee and billing rules, every diagnostic code with everyday condition names, and every error and explanatory code with what it means and how to fix it. They're free and need no account.
