Explanation
Updated October 26, 2026
Allergy hyposensitisation injection, billed per injection with a visit or as the extra injection after G212.
What it covers
G202 is a hyposensitisation (allergy) injection, billed for each injection (J4). When the injection is the sole reason for the visit, bill G212, which includes the first injection (J4).
If a patient comes for an allergy injection and also has an acute infection or another unrelated problem that would otherwise need a separate visit, you may bill the appropriate assessment as well as the injection (J4).
Billing rules
- Maximum 2 G202 when an assessment is payable for the same visit (J4).
- Maximum 1 G202 in addition to the injection included in G212 on a sole-reason visit (J4).
- G202 is not marked (+), so the G700 premium does not apply (J4).
- Insect venom immunotherapy is billed as G205, per injection, maximum 5 per day (J4).
- Payable on the same day as a counselling service by the same physician (GP58).
- FHO: a Schedule 2 core (in-basket) service (Agreement p.76).
Often billed with
- G212 on an injection-only visit, with one extra G202 for a second injection (J4).
- An assessment for an unrelated problem treated at the injection visit (J4).
Common rejections
- EQ2: The specialty code on the claim isn't one the physician has registered and active with the Ministry on the service date; check the specialty set up in your billing software.
- EH2: The health card version code on the claim doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit.
- VJ7: OHIP received the claim more than three months after the service date, so it was rejected as stale-dated (GP9).
- A3H: G202 is capped at 2 per visit with an assessment and 1 extra with G212 (J4), so billing more injections on the line rejects.
- A36: OHIP describes this as 'Claimed by Other Practitioner': another physician has already been paid for this service for the patient.
