K300A: Video visit code add (existing/ongoing Patient-Physician Relationship)
K300A is the no-fee indicator that marks a comprehensive virtual care claim as done by video. Required on the claim; video pays the full in-person fee.
Billing rules
- Video services are paid at the in-person fee (A70).
- Only Appendix J Section 1 codes can be billed by video. Consultations and C-prefix and W-prefix codes are video only (AJ1, AJ2).
- The platform must be on Ontario Health's verified list, and the chart must record that the visit was by video (A68).
- The visit must be patient-initiated or a medically necessary follow-up. Check-in calls, administrative calls and routine normal results are not payable (A68).
- If the visit switches between video and phone, only the modality used more than 50% of the time is payable (A69).
Common rejections
- VH8: The date of birth doesn't match OHIP's record for the health number. Correct it and resubmit.
- EH2: The health card version code doesn't match the Ministry's record. Get the current version code and resubmit.
- EQE: The physician wasn't registered in the group on the claim on the service date. A registration issue, not a coding one.
- R04: OHIP's description is 'service excluded from RMBS', the reciprocal billing system for patients insured by another province. The service can't be billed through reciprocal billing for that patient.
- EH4: The patient's OHIP coverage had ended before the service date. Confirm the health number, then the coverage.

K301A is the no-fee indicator that marks a comprehensive virtual care claim as done by phone. Phone visits pay 85% of the in-person fee (95% for some).

- Telephone pays 85% of the in-person fee (95% for K005, K007, K197 and K198). Bill the full value; OHIP reduces it (A70).
